s well acquainted with one Horton Cox Late Private Co A 12 or 6th Regt Tennessee Cav Vol in the [serace written over with] servace of the United States saw Horton Cox Enlisted under Capt Alfred J. Lane who was Recruting for the US sevace on or about the 29th day of March 1863 and was in the sevace on duty up to about the 31st day of July 1863 when he the said Horton Cox died at Nashville State of Tennessee of Dirarhea a dissease which he contracted while in the sevace and while in the line of his duty.
