Gives date of death as 6 Feb 1884 age 40 years 3 months 19 days which would have made his date of birth as 18 Oct 1843. Also says he was a druggist and died of pneumonia
Marriage certificate applied for on June 28, 1886 In Macon City, MO. This is to certify that Mr. James Forest and Miss Margret E. Bonnefield of Ten Mile Township, Macon Co. State of Missouri were united in holy matrimony at La Plata Ten Mile on the 15th April A D 1866 by me S. P. Van Dowyer, Minister of the Gospel. filed for record June 28, 1866
James first enlisted July 25, 1861 and was honorably discharged August 13, 1865. He is listed in the Company Descriptive Book with the following description; Age 19 years; height 5 feet 6 inches; eyes blue; hair light; Complexion fair. States he was born in Athens Co., OH and his occupation farmer. He enlisted at Downington, Ohio. He re-enlisted in 1864 and was paid a bounty of $60.00. Volunteer Enlistment as a soldier in the Army of the United States for 3 years in Larkinsville, AL on 15 March 1864 in which James states he was born in Athens Co., OH, aged 21 and was a farmer by occupation. Mustered into service on 18 Mar 1864 at Larkensville. On June 27, 1864 he is listed on the Casualty Sheet of wounded at Kenasaw Mt. GA. In June 1865 he requested a 10 day furlough to return to his home in Meigs Co. OH to attend to business.
James was mustered into the service in Company C, 30th Regiment of Ohio Infantry Volunteers on the 18th day of March 1864.
On the company muster roll for August 15, 1861 in Columbus, Ohio. In October of 1861 he was serving as wagoner. For March and April 1862, he was furloughed sick from Jan 21 to Feb 21, 1862 and rejoined his company March 3, 1862. During his furlough, he returned home. In March 1864, he was discharged to reenlist as Vet. Vol. in Larkensville. His second discharge was in Little Rock, Arkansas.
Margaret applied for widows pension. Declaration for Widows Pension under the Act of June 27, 1890. State of Missouri, County of Linn; on this 22nd day of July, 1890 personally appeared before me, a Notary Public within and for the County and State aforesaid, Margaret Forest, aged 43 years, a resident of the town of New Boston, County of Linn, State of Missouri, who, being duly sworn according to law, declares that she is the widow of James Forest who enlisted under the name of James Forest and served at least ninety days in the late war of the Rebellion, who was Honorably Discharged August 1865, and died at New Boston, Missouri. That she was married under the name of Margaret I. Bonifield to said James W. Forrest, on the 15th day of April, 1866 by Rev. Vandosser, at Tenmile, Macon Co., Missouri. That she has not remarried since the death of James W. Forrest. That she is without other means of support than her daily labor. That names and dates of birth of all the children now living under sixteen years of age of the soldier are as follows; Maritt A. Forrest, born March 28th, 1875 Addie E. Forrest, born July 18th, 1877 Glen A. Forrest, born September 18, 1879 Willard Forrest, born February 19, 1882 Annie M. Forrest, born March 23, 1884. That she has heretofore applied for pension and the number of her former applications is 338944. Margaret was given pension of $8.00 per month for herself, and $2.00 per month added for each child. She was last paid $25.00, to Feb 4, 1919 and was dropped from the roll because of death March 7, 1919, Arnett, OK. Certificate No. 300046. Several affidavits were filed in order for Margaret to receive the pension.
State of Missouri, County of Linn in the matter of Margaret I. Forrest widow of James W. Forrest late of Co. C, 30th Reg Ohio Vol. On this the 21st day of October 1887 Personally appeared Margaret I. Forrest before me a Notary Public within and for the afore said county duly authorised to administer oaths of Margaret I. Forrest age 40 whose PO address in New Boston, Linn County, Missouri who being duly sworn declares that she the calimant is unable to furnish further evidence in her claim for pension on account as follows; he came home from the war in 1865 to Burlingham, Ohio came from there to Missouri then back to Ohio in 1871 then from there to Jackson, Jackson County, Minnesota. He was treated while living there by a phsician that I cannot remember the name then moved from there to Newton Jasper County, Iowa in 1874 lived there two years then back to Missouri in 1876. I writen to the parties that treated him and cannot find them there whereabouts or place of abode. I further state that I have used every effort to procure said evidence and seems that I cannot during the periods of time which I have not already furnished. I further state that I am the widow of James W. Forrest. Statement signed by Margaret and L. B. Huessen, Notary Public.
State of Missour, County of Linn. On this the 7th day of February 1888 Personaly appeared before me the undersigned a Notary Publig within and for the county afore said Margaret I. Forrest widow of James W. Forrest an applicant for Pension and swears on her oath that she is unable to furnish medical evidence and treatment of her said Husband on account of him & her moving about a great deal of the time after he came home from the war for his health and the physicians who treated him a part of the time is dead, To wit: Dr. Bouton, Mayview, MO. also Dr. Jones of Pomroy, Ohio treated him, he is dead. He was also treated at Jackson, Minnesota the name of the Dr. I do not remember, therefore cannot find him, as he has left that place. Applicant further states that she is unable to furnish evidence in regard to the birth of William E. Forrest Born on the 7th day of September, 1873 while we lived in Jackson County ten miles from Jackson town or P.O. all the parties that attended the birth of the said William E. Forrest has left the county. I riten to the Post Master and others cannot hear from any one that was present. Applicant further states that she has used every effort to produce evidence in her claim and that the facts set forth are true to the best of her knowledge. Sworn to and subscribed before me this day by the above named applicant and I certify that I read said affidavit to said applicant and acquainted her with its contents before she executed the same. I further certify that I am in to wise interested in said case and that the applicant is at this date the widow of James W. Forrest as represented and that she is a creditable person given under my hand and seal this the 7th day of February 1888. I also was present at the soldiers death and funeral on the 7th day of February 1884. L. B. Huessen, Notary Public.
Physicians Affidavit dated July 15, 1887 by Dr. D. V. Howard; I have known the soldier for about ten years and did not know him until 1876, about that time he moved to this area have been his family physician ever since. I treated James W. Forrest - on or about April 1877 for nervis Prostration with Paralysis with Pain in side and hip, also I have treated him almost every month from that date to the date of his death. I further state that to the best of my knowledge he died from the effects of the wound caused as stated to me by him of an explosion of a shell while in line of duty, Nevertheless I state that he died from the effect of above stated. I further state that I tended on him in his last sickness which was phenumonia, nervous prostration with paralysis.
Physicians Affidavit; That I have been acquainted with the said James W. Forrest for about seven years prior to his death, was his family physician treated him during said time until his death Feb. 7, 1884 and during his last sickness for pain and disease of his side which he said was caused by an explosion of a shell while in the service in U. S. Army. He died from a disease of left side resembling pleurisy was present at the time of his death and treated him during his last sickness. General Affidavit dated 10 Sept 1887 My L. L. Miller: I have been acquainted with J. W. Forrest some three years from 1873 until 1876 his health was quite poor not able to do full days work in that time. Troubled with heart and lung disease. Same affidavit M. W. Atwood; was acquainted with the above claimant J. W. Forrest that he lived upon my farm as a neighbor for the time above stated by first affidavit and that the statement setforth above are the facts of his condition for the time named above.
General Affidavit dated 13 Oct 1886 given by Mrs. M. E. Miller, age 53; I was present at the birth of Merritt Forest March 28, 1875, also Mrs. Rebecca Hamilton, Mrs. James Chadworth acted as midwife. He was born in Palo Alto, Jasper Co., IA.
Affidavit dated 28 May 1886 given by James. H. Baker; I was in the charge on Kenasaw Mt. June 24, 1864, with comrade James W. Forrest of Co. C. 30th Reg. I belonged then to the same company. I did not see him at the time he was knocked down by the explosion of the shell but saw him the same day in the evening after the charge, and found him suffering great pain from the wound in his side and hip. It did not appear that any part of this shell had struck him that the concussion whirled him around, knocking him down and as I thought from his walk afterward partially paralyzing his hip. I have heard him complain often afterward especially after a fatiguing march of much pain in his side and hip.
Affidavit dated 11 March 1887 given by James H. Wilson; I was the 1st Sergeant of Co. C, 30th Ohio Infantry. Knew the deceased from the close of the war until 1873. James W. Forrest was a private in Co. C, 30th Ohio Volunteer Infantry. The deceased was injured in the right side by the bursting of a shell in 1864 during the battle of Kenasaw Mountain. The bursting of the shell at the time seemed to partialy paralize his right side. Has never since that time been able to perform a days work.
Proof of Disability statement given by John A. Hawk, 11 July 1886 states that he was acquainted with James W. Forrest, applicant for Invalid Pension and knew the said James W. Forrest to be the identical person of that name who enlisted or volunteered as a Private in Company C, 30th Regiment of Ohio Vols. That said James W. Forrest, while in the line of his duty, at or near Kenesaw Mt. in the State of Georgia did, on or about the 27th day of June 1864 become disabled in the following manner, viz: that while charging the emenies works at the base of Kenesaw Mt. a shell exploded wounding him in the side the flesh did not appear to the mangled but paralized. That the facts stated are personally known by reason of his having been with the command at the time and saw applicant immediately after he was wounded.
James is listed on the Company's casualty sheet, place of casualty not stated, nature of casualty; wounded slightly, date of casualty; June 27, 1864.
Affidavit dated 9 Nov 1887 given by John A. Forrest, age 40, living in Hope, Dickinson Co., Kansas: I have been with James W. Forrest at intervals from the time he came home from the army. When he came home he was complaining of a would in his side which he received while in service of the U. S. I will further state that I am a brother of James W. Forrest but have no interest in the above claim except brothers children and widow. Every time he takes cold he would complain of the wound in his side (can't read rest of statement)
Affidavit dated 8 Dec 1890 given by E. E. McKinney and G. W. Nagel; As to whether either the claimant or her husband James Forrest was ever married previous to marrying each other we are confident they were not but have no way of knowing corretly. Whether claiment has married since soldiers death, no. The children are all still living. The claiment supports herself and children by labor. Her property amounts to almost nothing, not over one hundred dollars in value and her income is a scant living. Our means of knowing the above is that we have lived very near neighbors since some years before James Forrest's death.
Affidavit given by Elizabeth Ellis on 3 Feb 1888 states that she was present on the 7th day of July 1871 at the birth of Mary N. Forrest and personally knew that she was born to James W. Forrest and Margaret I. Forrest.
Affidavit givedn my John P. Herr in Larayette Co., MO dated 6 Sep 1886; Knew the parties well, he had been a Union soldier from statements to me corresponding to historic facts. His wife had a child born to her while they lived here about 12 months altogether. Came in the fall of 1870 AD to my personal knowledge because I was the first he consulted with and I rented him my house.
Physicians Affidavit given by Dr. D. V Howard given 21 May 1887: In the pension claim of Margaret Forrest widow of James W. Forrest. He has given medical advice and treatment to James Forrest as follows, For disease of the side said to be caused by wound while in the United States Service treated him frequently during each year from 1877 unti his death for said disease pain in side and nervous prostration and to the best of my knowledge said soldier was not able to perform manual labor more then one fourth his time. Also attended on soldiers wife at the birth of the following children which are yet living, to wit: Adela E. Forest born July 18, 1877, Glen A. Forest born Sept 18, 1879, Willard Forest Born Feb 19, 1882, Anna M. Forest born Mar 23, 1884.
Medical Division Bureau of Pensions, Washington D. C. May 12, 1888. Claim No. 338944, Claimant Widow, soldier James W. Forrest, Co. C. 30th Reg. Ohio Vol. Inf. Respectfully returned to Mr. Van Martin Cheif of Board of Review with the opinion that from the evidence on file soldiers death cause probably acute disease of longs and cannot be accepted as a result of the injury incured in the service.
Letter written by Margaret: Arnett, OK, Jan 6, 1917. Bureau of Pensions, Office of the Disbursing Clerk, Washington D.C. Sir, I am 70 years old to day. I see I am now entitled to a Increese of Pension. What must I do to get it, pleas let me know. Margaret I. Forrest Pension Certificate No 300046. Adress Margaret I Forrest Arnett, Okla. Rout # 3. I sent order for change from Tolas, N. M.
Letter written by Mollie: Arnett, Okla. 4-16-1919 Bureau of Pensions, Our mother Margaret I. Forrest - Holder of Pensin Certificate No 300046 died March 7, 1919. Just 1 month after she drew her Pension Feb 4th now will her children be entitled to that one month Pay $25.00 due her if so please notify me as to wht I shall do to get it. Yours Resp. Mrs. Mollie Crail, Arnett, Okla. Rt. 3
Widows Pension Claim: Approved for rejection on the grounds that the cause of the soldiers death disease of lungs, was not due to the injury from explosion of a shell, nor claimed or shown to have been of wounds due to his military service. Dated May 16 1888.
Margaret originally filed claim in May 1886. Her claim was finally approved and she recieved payment for herself and children Mary, William, Marritt, Addie, Glen, Willard and Annie ($2.00 per month for each child until each reached the age of 16 & $8.00 per month for herself. $22.00 per month total to raise 7 kids). At age 70, she received $25.00 per month for herself.
