Jack lived in Louisville, ca 1947-1970 then
moved to New Albany Ind where he lived until 1993, then moved next to daughter
in Louisville (Martha lived 9007 Court,40222 he moved to 9003 Dove Creek Ct
Louisville 40242, )Placed in Nursing home ca 1 Jan 1996, and was on Oxygen
for enphysema caused by smoking heavily most of life, - wanted to die to
"go see Mamaw, and Papaw, and Jessie and Jesus" took off Oxygen mask
several times, always put back on by nurses until Larry and Martha both
came and stayed to be sure that they did not do that again, and he died
3 AM March 13 1996, rather long drawn out, - note nurse came in and tried
to put back on and he and both ch said NO emphatically, or she would have
done it again- just withdrew "extreme and painful measures" to keep him
alive per their thinking.
